I think she forgot that breastfeeding only has contraceptive effects if it's 100% exclusive, on demand and every couple of hours. I suspect baby Hatties has a pacifier and possibly some formula supplements. Even if you are 100% breastfeeding though you have to be careful in the first 6-8 weeks after birth, if not abstaining, as it takes several weeks for your body to heal and recover. At 4 weeks pp sex was the last think on my mind!!
