To soak or not to soak?

I am a recent reusable convert and want some advice on washing nappies:
1) Should i dry pail used nappies or soak them? If soak - with what (eg napisan)?
2) Should I wash them with all the pads etc attached or separate them out?
3) Should I wash them in the mesh bag?
4) How do I get poo-stains out without boiling/high temp wash as this defeats the environmental effect?
