Luteal phase defect and PCOS? What to do?
Hi all, hope you're having a lovely weekend
Posting this in the PCOS forum too - sorry if breaking any etiquette, let me know if so!!
I'll try to keep this brief as it could end up being quite a long story
Background
-Referred to gynae for long cycles/spotting post pill. Confirmed PCOS. Blood tests showed raised prolactin (due a retest in a few wks)
-Short (10 day) luteal phase with spotting/bleeding from 2 dpo onwards till AF
I think I have a luteal phase defect but my gynae brushed this off and has just suggested putting me on metformin for PCOS. He has not tested for progesterone as doesn't think it's worth it. I'm not keen on trying metformin yet as my last two cycles have been normal.
Was wondering about trying to regulate hormones and increase LP/reduce spotting with Vit B6 and Agnus Castus - heard both can help
But scared of self-diagnosing and taking supplements that I don't know much about
So
-Do you think I could have an LPD?
And what the hell do I do now:
-Go to my GP and check it's ok to take the b6/AC?
-Go straight to gynae again and ask him to do more (perhaps he'll prescribe progesterone supps?)
-Book an appointment with fertility specialist?
-See a medical herbalist about B6 and Agnus Castus - no idea if this is something people do

So went back to the gynae and he didn't want to give me progesterone, as he said it wouldn't be helpful (still confused about this tbh!)He was keen I went on the Metformin, so, after consulting a GP friend too, I went for it.I've been on it for a couple of months now and haven't seen any difference. Post ov spotting/bleeding, if anything, is worse
On the plus side I've dropped half a stone without trying (didn't actually need to lose any weight, but not going to complain about dropping a dress size!)Been eating for England over the last few weeks, just got on the scales, preparing for shock and OMG, down another few lbs!
Also was prone to cystic acne, and hardly have any spots now - yay for unexpected benefits!
Haven't been back to have my prolactin tested as wanted to give the metformin a chance to work first (gynae said it might lower prolactin). Will go back this month though.
I was wary about taking extra vits but decided to take 50mg vit b complex this month. Am hoping to ov soon, so will let you know what effect it has, if any....fingers crossed x
Sorry to hear things havent improved for you but have my fingers crossed that they get better.
I went to see my doctor earlier this week. She took some blood to test my progesterone levels - if they are low it means you can spot before your period is due. She took the test 7 days after I was supposed to have ovulated and it came back normal. I'm going to make another apt with her next week to see what she says. I have heard that the contraceptive pill can make you deficient in vit B6 - but I came off it nearly 1 yr ago - I asked my GP if supplementing can help lengthen my luteal phase and she said not to go above 50mg per day because of the risk of excessive amounts causing nerve damage. I'm still tempted to start taking tablets for 50mg though.
If I were you I would personally go back to your GP and explain that things havent improved because they might be able to give you something else that will give better results. I would also mention that you have lost weight without dieting (if its a lot) just to give them the full picture of whats happened since you last saw them.

I also went back to gynaecologist and told him about spotting. He has now prescribed me progesterone (Cyclogest) to take from ovulation for 14 days to help with the spotting. This is without any progesterone blood testing - he doesn't believe it is accurate or worthwhile.I have no idea about this!?!
I just feel totally :? by it all to be honest. I hope the cyclogest will help but starting to despair a little...I know I should feel like I've made some progress this month but instead I'm just thinking how many more things will we have to try before this works out. Feel like I'm at the bottom of a very, very big hill.
Sorry to hear about this. If it makes you feel any better my GP has reassured me that you can still get pregnant if you have light spotting - although she said it isnt normal to spot right the way to your period like we are. I am due to go to a specialist soon so will let you know what they say. I had my progesterone measured 1 week after my luteal phase surge and it came back at 42.6 which my GP said is good - she seemed confident that I am ovulating however after what your gynae said I'm not so sure now. I'm sure they know what they are talking about - I would go for what ever they suggest - let them worry about it and try not to stress too much over it yourself. I know its hard but I think if you try and be as positive as possible about it then that will boost your chances. I have done a lot of reading about this and one of the things I have read is that stress is meant to be a factor that can cause irregular menstrual cycles/ spotting - not sure if that is true or not.
Keep your chin up - if the cyclogest doesnt work then I expect your gynae will try something else until they find the right treatment that works for you xx
Wish you luck - think this just shows our bodies work in v strange ways and we probably drive ourselves mad second-guessing stuff all the time (although I know I will continue to do so!)x
