nhs maternity exemption cert

when every body got theres did you have to apply for it?
i got mine this morning and they spelt my name wrong but it was the most stupid mistake ever but i got mine automaticly;\)

    midwife filled in the form for mine at around 8 weeks, and i got it not too long after.

    suppose i really should get mine changed to married name now...but i think it can wait till after baby is here..i'm not giving up my card ...i need it to get my gaviscom!!

    well i just phoned em up 2 tell em they got it wrong and shud get another 1 in next 7 days
