maternity notice for employer

Hi Can anyone please help. I am trying to write a letter to advise my employer when I am due to start matty leave, when I intend to return and of my intention to reduce my hours when I return. Does any1 have a draft letter that I could have a look at and perhaps copy. Im really struggling with this. Thanks
