sure start grant form

hi i applied for the sure start maternity grant and sent the form away and got it sent back today because on back where midwife was to write in her number (i guess her registration number or whatever), she didnt fill in the surgery address, and phone number. could i just fill this in myself and treturn it to them? (is it just my local surgery/hospital or does that mean its her head office or employer or whatever?
