Lining or miscarriage

Hi I’m new here I’ve had two older children I have pcos thyroid problems and diabetes I’ve been trying to get pregnant for three years recently I missed my period and got very bad nausea I decided not to do pregnancy test as they always negative Then I got the worse pain I’ve ever had and very heavy bleeding almost went to hospital then I passed this it was small does anyone know what this is ? Thanks
