MATB1 Form

Hey!
I went to my midwife last week. Im now 26+5!
I asked her for my MATB1 form and she replied by sayin the there wasnt a single one in the building!
Does anyone know the latest that your employer is allowed to accept it as i am worried that i'll be gettin mine too late cos i now have to wait til my 28 week appointment xxxxx:cry:

    you should call your surgery- you dont need to see your mw to get it they should be able to arrange for you to pick it up from reception x

    Hi,
    I think your employer should have the form around 15 weeks before your due date, so probably should have had it last week. MATB1 forms are usually given out at 20 weeks. Like michmummy said you should be able to pick one up from your surgery so it's worth giving them a call and asking for it to be left at reception for you. xx
