Lunch time supervision of reception children

Is there anyone who feels their children are not getting much supervision during break times. There have been 2 incidents in 2 weeks, that I know of,  during lunch time and when I ask for more details from school staff to understand what happened no one ever seems to know or even have witnessed it, however the children have reported it. I appreciate it s a busy time for the school, but surely the younger kids should be monitored a but more.
