I'm not saying it's time for hormones or anything, but many trans people know that their gender does not match there body as young as 2 years old. ( Trans kids are estimated to be from 0.5% to 0.2% of all kids depending on the study).
In my day I had know idea why I was different, but I always knew I was. I didn't figure it out til my mid 20s.
Either way, the best way to deal with this is to let "john" be himself. If it persists, then using male pronouns is appropriate and starting to look for a WELL QUALIFIED child psyc. (not just someone who THINKS they know what they are talking about) is a good idea in case he does turn out to be transsexual or otherwise gender non-conforming.
