I wouldn't panic - most babies don't sleep through that young! It's perfectly normal for a baby for wake up for feeds during the night until they're much older than that. In fact, some still wake until over a year old! X
I'm having the same problems.. its really worrying at times.I've found that putting on sounds of nature really helps.. Like the sound of rain, birds singing etc.. I found these really work!

I know some babies sleep through but isnt Common at 16 weeks a lot is going on with growth spurt and sleep regression as their sleep pattern changes
they all sleep through at their own time despite what you might hear from other mum’s
hang in there you are doing great
